Dr. Phillip Weeks, PharmD, FHFSA, AACC, BCCP is a board-certified clinical pharmacist with more than a decade of experience in acute care of patients with cardiovascular diseases, and he currently serves as a Clinical Pharmacy Specialist in Advanced Heart Failure and Transplant at Memorial Hermann - Texas Medical Center in Houston, Texas. His areas of focus include medication management of patients with heart failure requiring mechanical circulatory support or heart transplantation. His additional clinical areas of expertise include the management of antiplatelet therapy, anticoagulation and vasoactive medication support.
